TERMS AND CONDITIONS AGREEMENT

I agree to the following terms and conditions:

Payments and Policies:  Class enrollment is based on a commitment to our fall through spring 10-month class schedule.  Pricing is based on the annual tuition cost.  Tuition remains the same each month regardless of long or short months and/or the number of absences. 

• An annual registration fee per student is due at the time of registration, which covers administrative costs, student liability insurance, and reserves your place in class. This fee is only charged once per year and is non-refundable.

• There are discounts for paying in full at registration, or in two (September and February), or three installments (September, December and March).  There is also a sibling discount.

• Each year we have an annual recital at a local theater. Participation in the recital is not mandatory, but we encourage all students to participate. The recital is a big event and it allows the chance for parents, family members and friends to watch the student perform and appreciate what they have learned.

• A non-refundable costume deposit of $35 is required in March with balances due in April. Note: It is our studio policy not to hand out costumes until balances are paid.

• Dancing Plus Gymnastics (DPG) will issue invoices by email which can be paid online.  Once the first month's payment and registration are paid, payments are set up as 9 recurring payments.  We can also arrange to bill as 9 separate monthly payments, if necessary.

• If payment is not received by the 2nd class of the month, a $25 late fee will be charged and the student will not be able to participate until the amount due is paid in full.

• If paying by check, make check payable to Dancing Plus Gymnastics.  A $25.00 returned check fee will be charged for any check returned by the bank for insufficient funds. After the first returned check, DPG will require cash or credit card payments only.

Refunds: There are no refunds on tuition, costume fees, class pictures, tickets, or workshops after September 30.  Wherever possible, it will be applied as a class credit.  There are no refunds for missed classes. Parents are responsible for making sure their child makes up any hours they may miss.  Make-up classes must be completed within three weeks of the absence.

Studio Rules: Students must wear proper attire (dancers: leotard, tights and dance shoes; gymnasts: leotard, shorts or stretch pants and no socks or shoes) and follow studio rules at all times.  If the student does not have the proper attire for class, they cannot participate.  In addition, students must be on time for class.  If there is an absence or delay, please contact the studio.

Please do NOT send your child to class if the following apply:

* They are waiting for results of a Covid-19 test

* They have been required to quarantine for any reason

* They are sick for any reason

PHOTO RELEASE AND AGREEMENT

I grant DPG the unrestricted right to publish and use photographs, images, and video of my child(ren) for advertising, publicity or promotion in any media and release DPG, its ownership, personnel, successors, affiliates and assigns, from any claim, demand, cause of action or proceeding of whatever nature arising out of any use, publication and/or distribution of any photographs, images or video in any media in accordance with the terms of this authorization. 

I understand that I can revoke my agreement by notifying DPG in writing of my decision.

RELEASE OF LIABILITY AND WAIVER​

I certify that my child(ren) is/are in good health and has/have no physical, mental or emotional problems that would interfere with participation in the program.

​As parent/guardian of the above mentioned participating child(ren), I am fully aware of and understand the risks inherent in dancing, gymnastics and other physical activities, including the risk of serious injury. I am also aware of the risks of a contagious illness such as COVID-19. I hereby waive and release DPG  and its staff from all liability for any and all damages, injuries, illness or loss of property incurred by me and my child(ren) during the course of instruction or participation in any DPG related activities. I further agree to indemnify and hold harmless DPG and its staff, from any liability arising out of negligent or intentional conduct of students, parents, family members, or other parties invited upon the premises by the student or any family member, which results in loss, injury, damage or illness to any other party.​

Verification: I hereby verify by clicking below and typing my name as my signature that I have read and fully understand and accept the information above and that by clicking accept, I am agreeing to this waiver and release of liability.
